7 Early Signs Your Kidneys May Need Attention

June 27, 2026 by Randi Shannon

More than one in seven adults have kidney disease — and the majority don’t know it. That’s because early kidney trouble is famously quiet. But “quiet” isn’t the same as “silent,” and the body is always talking. You just have to know how to listen.

Here are seven signs worth noticing — the first three are the ones I see missed most often.

1. Unusual fatigue that rest doesn’t fix

When the kidneys struggle, waste can build in the blood and the body makes fewer of the signals that keep your energy up — including the one that tells your bone marrow to make red blood cells. Tired-all-the-time has many causes, but it’s worth ruling this one out, especially if the fatigue came on gradually and sleep doesn’t touch it.

2. Changes you can see — swelling or puffiness

Subtle fluid retention, often around the ankles, feet, hands, or under the eyes in the morning, can be one of the earlier outward signs that the kidneys aren’t clearing fluid as they should. Sock lines that stay dented longer than usual are a classic early clue — and puffy, dark under-eyes are one of the first things I look for in a face analysis.

3. A history that raises the odds

Diabetes and high blood pressure are the two biggest drivers of kidney strain. If either is part of your story — or if kidney disease runs in your family, or you’ve had years of regular pain-reliever use — your kidneys deserve attention before symptoms ever appear. This isn’t a sign you feel; it’s a sign you know.

4. Changes in urination

Getting up at night to urinate more than you used to, urine that’s persistently foamy or bubbly (a sign of protein), urine that’s darker or lighter than normal, or simply going more or less often. Any of these on their own can be nothing; together with other signs they’re worth paying attention to.

5. Persistent itching or dry skin

When the kidneys can’t keep minerals and waste in balance, the skin often shows it first — unexplained itching, especially on the back or legs, and dryness that lotion doesn’t fix.

6. A metallic taste, nausea, or loss of appetite

Waste building in the blood can change how food tastes and make you queasy, particularly in the morning. People often describe a metallic or ammonia-like taste and a loss of interest in meat.

7. Muscle cramps and trouble concentrating

Shifts in calcium, phosphorus, magnesium, and potassium can show up as cramps, especially at night. Brain fog and trouble focusing often travel with the fatigue described above.

What your tongue, face, and nails are showing

This is where I start with every client. In my signature face, tongue & fingernail analysis — a 5,000-year-old observational art — kidney strain is one of the clearest patterns there is: puffiness or darkness under the eyes, a pale or swollen tongue with a thick coating or teeth marks along the edges, and nails that are pale, ridged, or show a white band at the base. The body shows what it’s carrying long before a lab result changes, and that early window is where root-cause healing has the most to offer. Read more in what your tongue can reveal about your health.

What to do if you recognize yourself

Find out where you stand. Two simple tests — an eGFR blood test and a urine albumin-to-creatinine ratio (ACR) — tell you what your kidneys are doing, and if you have diabetes or high blood pressure you should have both every year whether you feel anything or not. Then come see me, because a number is only the beginning of the story.

The earlier kidney strain is caught, the more there is to do about it — and that’s where a whole-body, root-cause approach has the most to offer. Stage 1 and 2 kidney strain is almost never caught by symptoms; it’s caught by people who decided to look. The body was designed to heal. The sooner you give it what it needs, the more it has to work with.

Frequently asked questions

What are the very first signs of kidney problems?

Usually nothing you can feel. The first measurable sign is protein in the urine. The earliest signs people notice tend to be fatigue, puffiness under the eyes or around the ankles, changes in urination, and changes in the tongue and nails.

Can you feel kidney disease in your back?

Chronic kidney strain rarely causes pain. Flank pain under the ribs usually points to something else — a kidney stone, an infection, or a muscular issue — and deserves prompt attention, but it isn’t a typical sign of chronic kidney disease.

What tests check kidney function?

An eGFR (estimated glomerular filtration rate), calculated from blood creatinine, and a urine ACR (albumin-to-creatinine ratio). Both are routine and inexpensive.

If I have these signs, what should I do first?

Get the two tests so you know where you stand, then let’s look at the whole picture — what’s causing the strain, what your body is showing, and what it needs to heal. That’s the work I do.
