Serious — and still full of meaningful choices
A stage 4 diagnosis (eGFR 15–29) is serious, and it’s frightening, and you deserve straight talk: this is advanced kidney disease. But “advanced” does not mean “out of options.” Not for how you feel, not for the rest of your body, and not for the direction you’re heading. I have never met a body that wasn’t still trying to heal.
As Traditional Naturopathic Doctor Randi Shannon, I provide whole-body, root-cause support for people at stage 4 — focused on what is driving the strain, on nourishing the body that’s carrying it, and on feeling as well as possible — while your medical team continues to monitor your kidneys.
What stage 4 means
Your kidneys are doing somewhere between 15 and 29 percent of the filtering a healthy pair would do. At this level the body begins to show it: fatigue, swelling in the legs or around the eyes, nausea or a metallic taste, poor appetite, itching, muscle cramps, trouble sleeping, or changes in how often you urinate. The kidneys’ other jobs — balancing minerals, supporting red blood cells, activating vitamin D — are under strain too.
It’s also the stage where the medical conversation turns toward planning: dialysis, transplant evaluation, more frequent monitoring. Those conversations are hard. They are also not the whole story. Stage 4 is not stage 5, many people spend years here, and how those years feel — and where they lead — is not fixed.
Where whole-body healing fits at stage 4
The medical model monitors and manages the kidneys. My work is everything the kidneys depend on — the parts of your life and your body that fall through the cracks between appointments:
Reading your body
My signature face, tongue & fingernail analysis shows me where the strain is and what the body is asking for — the tongue, the eyes, the nails, and the skin tell a story the lab report doesn’t. Every plan starts here.
Feeling better day to day
Fatigue, nausea, poor appetite, restless sleep, itching, and brain fog are the things that make stage 4 exhausting, and they are rarely the focus of a short medical visit. We work on them directly, with food, minerals, hydration, herbs, and rest — chosen for your body.
Eating well inside a tighter picture
By stage 4 the diet gets more careful. Most people are handed a list of what they can’t eat and left alone with it. My job is helping you build meals you actually enjoy, that nourish rather than burden, and that don’t quietly add inflammatory or chemical load. Real food, prepared simply, matched to your body.
The causes underneath
Diabetes and high blood pressure drive most kidney disease, and they keep driving it at stage 4. Inflammation, mineral depletion, a congested liver, a struggling gut, and toxic load are usually in the picture too. Addressing what is actually causing the strain is the heart of root-cause work, and it protects the heart as well as the kidneys.
Removing what’s working against you
Supplements, “kidney cleanses,” over-the-counter pain relievers, and well-meaning internet advice can be harmful at stage 4, where the margin for error is small. One of the most valuable things I do is help you sort what belongs in your body from what doesn’t.
Preparing your whole body for whatever comes
If transplant is on the table, the stronger and better-nourished your whole body, the better the road. I provide whole-body support for people waiting for a transplant and support for people on dialysis, so if either becomes part of your story, you won’t be starting over.
Honest hope
I will not promise you a specific lab result — at stage 4 especially, anyone who does is not being honest with you. What I can promise is a knowledgeable, compassionate traditional naturopath who treats the whole you, who believes in the body’s ability to heal, and who has spent 25 years watching people at every stage feel better than they were told they could. Your medical team keeps watching the numbers. I help the rest of you heal.
Frequently asked questions about stage 4 kidney disease
How long can you stay at stage 4 kidney disease?
There is no single answer — it depends on what’s causing the strain, how well the causes are addressed, your age, and the rest of your health. Some people progress quickly; many remain at stage 4 for many years, and the difference is very often what they do about the causes. Your nephrologist can tell you how your numbers have been trending; I can help you change what’s driving them.
Will I need dialysis at stage 4?
Not necessarily, and not yet. Dialysis is a stage 5 conversation (eGFR below 15) or one that symptoms force. Stage 4 is when your medical team begins preparing, so that if it ever becomes necessary it isn’t a crisis — and it is also when whole-body healing work matters most.
Can a naturopath help with stage 4 kidney disease?
A traditional naturopath helps with how you feel, how you eat, the causes underneath, and your body’s overall resilience. That is complementary to the monitoring your medical team provides, not a replacement for it. I don’t make claims about outcomes; I do this work because of what I’ve seen it do.
What should I avoid at stage 4?
Processed food, excess salt and sugar, chemicals and additives, NSAID pain relievers unless your doctor approves, and any supplement or “kidney support” product that hasn’t been chosen for your body. Bring everything you take to our first session and we’ll sort it together.
Why do I feel so tired at stage 4?
Several things stack up: waste building in the blood, low red blood cells, poor sleep, a restricted diet that’s often short on real nourishment, and the emotional weight of the diagnosis. Each of those has something that can be done about it, and that is exactly where we start.
